I suggest watching the Japanese version if you're not picky with reading subtitles during an anime. It's much, much better.

Anywho, if you mean the first arc he was never really "drugged". He was hallucinating from Oyashiro's curse and thought that Mion had a needle in her hand when in reality it was a marker for his penalty game. He went beserk, killed both Rena and Mion and then clawed his throat.

 

... That makes... SO much... sense! So thats why there was no syringe... right? Behind that clock? Then what would be that ripped out middle part of the note? Hm... Good storyline, this Anime.
