I use...
Fatality cam - to see what do your enemies do after they kicked your butt :
Chase Camera Mod - makes third person mode less laggy and more usable
Attack and hide - for sneaky characters, it's kinda needed cause regular close combat characters are so much stronger than us tiny assasins...
Mighty bows - same but for archers, must have if archery is your primary ability
Adrenaline Oblivion - makes oblivion more dangerous and crazy :
Key Chain - you want it, trust me
Enchantment Restore - restores energy of magic items like it did in morrowind

That's about it, though cloudboi aka genocidalgeorge is near by...never seen him thankfully.

 

Any downsides to these MODs? Are they hard to install? Do they drastically change everything like Diablo II's MedianXL? Where can I download them? Where's the cake?

 

Also, I use some more mods, but those are just for fun, not improving the gameplay.
Such as:
Fenrir the wolf - THE BEST companion mod out here (it's also fun to get custom textures for your wolf, they are also downloadable)
Custom races
Body replacements - cause bethesda makes ugly models


Oh, and you do want Operation Optimization, even if you have powerful comp.

Easy to install. No downsides, u can remove them. Only Adrenaline Oblivion changes the game in more drastic way, but I still reccomend it because it has few modes. I use only this one which makes animals such as wolves behave more like real wolf should - hunt in packs and appear in bigger numbers far from the roads...
