PS2 game comes in .MDS format?

    do i have to convert it manually or does alcohol 120% do it automatically?

    .mds is alcohol format.

    so it burns it automatically to an ISO or do i have to convert it to ISO first.

    Avatars were reset, he just hasn't uploaded one.

    MDS/MDF can be burned to a CD using Alcohol or IMGBurn or probably other image burning softwares.

    If the file is more than 700mb, then yes.
