I suppose if a premium member wants a reasonable custom title then that'd be fine. Does not look like the forum support it.
I don't even think we have custom titles enabled at the moment. Either that or that particular option is the only one that not even I have access to in user profiles.
Global Moderators can change any aspect of your account. So I think they can change custom titles as well...though I'm not positive on that. I just know the powers of an admin account. I've only ever been a user and admin. Never a moderator.
I just checked and it doesn't appear as if SMF even supports this. So unless there's a mod it won't happen.

On another smf forum which i help moderate (yeah of all people XD) they have custom titles. But it's just individual ones made on the spot, so it's made the same way a normal title is made. All you could do is make a new "rank", set the permissions, and just accept the donation as normal. I think rock91 would be a better person to ask about this sorta thing, he used a free points system which I don't...actually know how to run XD
The only problem about that is that I have no idea how he did that (since he never tells me how to do things in the ACP) and he's in Romania without internet for a month or so.
Well at least consider the possibility of just manually making titles and just paying for them like any donation if we can't get the point system... That was free, and from the looks of it, manually worked.
User groups is not an option, it is a lot of work to make one and it slows the forum down. RomUlation has 1.6 million members so we have different performance concerns, there are some things which small forums can do which we can't.
