It's generally an American thing I think... Us Europeans have better ways to insult people... Isn't douche the french word for shower ? It's something along those lines anyway...
Douche is short for Douche Bag. And as bloodvayne posted, Its a slang term used to call someone Stupid.
not sure about french, but it is extremely similar to portuguese, you only need to take out the ''o''.
